Great clinical outcomes don’t happen by accident. They happen when teams identify a need, bring the right disciplines together, try something innovative, and use data to show that their work is making a difference. The ONE Clinical Trailblazer Award is an opportunity for Ensign-affiliated teams serving skilled nursing, long-term care, or both to showcase that work and celebrate clinical innovation that delivers measurable results. And there’s more than recognition on the line: $15,000 for first place, $10,000 for second place, and $5,000 for third place.
What Makes a Trailblazer?
Start with a clinical need and the data that helped you identify it. Teams can use Power BI, the ONE Clinical outcomes dashboard, SimpleLTC, or another report to establish their baseline. From there, show how your interdisciplinary team responded, what each discipline contributed, what made the approach innovative, and how you plan to sustain the change.
Submissions should include at least 90 days of baseline-to-current results demonstrating the impact of the initiative. Reports, graphs, and dashboard screenshots can help tell the story.
Teams will also submit an organization-accessible Video Stream or SharePoint video of five minutes or less, highlighting the challenge, solution, human impact, and results. If residents or family members appear in the video, signed consent must be kept on file.
Take Your Success to the Next Level
The story doesn’t end with what you’ve already accomplished. Applicants should explain how award funds would help extend their success, including itemized estimates and the outcome the team plans to measure next. Facility teams should also secure ED and DON review and approval before applying.
